US Defense Secretary Pete Hegseth has announced a sweeping new health directive that will require mandatory screening for testosterone deficiency for all US military personnel aged 30 and older, integrating the tests into routine annual physical examinations.

Hegseth shared the initiative in a video posted to his social media platform X, which he captioned "High-T Department." In the footage, the Secretary explained that he is authorizing the screening protocol to ensure that troops maintain optimal hormonal levels, thereby guaranteeing they can "operate at their absolute best." Service members identified with clinically low testosterone will be offered voluntary hormone replacement therapy. While the test is compulsory for those over 30, participation remains optional for personnel under that age threshold.

Addressing potential misconceptions, Hegseth drew a clear line between the program and illicit performance enhancement. The military currently enforces a strict prohibition against taking testosterone for non-medical reasons, such as artificial muscle building without a valid prescription. Hegseth emphasized that this new initiative is explicitly "not about artificial enhancement" but rather a genuine health intervention.

However, a significant point of ambiguity remains regarding gender inclusivity. The Secretary did not specify whether the mandatory screening would apply to female service members, despite the fact that testosterone levels also naturally decline in women as they age. When pressed by reporters on whether female troops would be eligible for estrogen-based evaluations or therapies as they enter perimenopause, the Pentagon declined to offer any additional commentary beyond the statements provided by Hegseth and Press Secretary Sean Parnell.

Parnell elaborated in a subsequent statement that the protocol will take immediate effect for all active-duty and reserve component personnel aged 30 and older. He asserted that establishing a comprehensive baseline of hormone health will allow the Department of Defense to deliver targeted therapies, ultimately sustaining a "healthy, capable, and decisively dominant fighting force."

The directive has already ignited a sharp political response on Capitol Hill. Democratic Senator Tammy Duckworth of Illinois, an Iraq War veteran and a member of the Senate Armed Services Committee, urged Hegseth to ensure hormone testing is made available to both male and female troops, advocating for equitable healthcare standards across the force. Conversely, Congresswoman Chrissy Houlahan, a Pennsylvania Democrat and Air Force veteran, swiftly dismissed the move on X, deriding it as Hegseth's "latest culture-war obsession," suggesting the policy is driven more by ideological posturing than pure medical necessity.

Despite the partisan divide, medical professionals have acknowledged potential benefits. Dr. Mohit Khera, a urology professor at Baylor College of Medicine who led a Food and Drug Administration (FDA) expert panel examining screening protocols and testosterone use in the military just last year, spoke to the BBC. Khera strongly advocated for universal screening for men over 30, pointing out that testosterone levels serve as a critical biomarker for assessing overall current health and predicting future medical risks.

He noted that low levels could disadvantage combat troops by adversely affecting muscle mass, bone density, and daily energy—all crucial factors for those serving in high-stress environments. Nevertheless, Khera added a vital medical caveat: physicians must exercise extreme caution and ensure patients present relevant clinical symptoms before prescribing replacement therapies. He warned against indiscriminate supplementation, stressing that treatment should remain anchored in symptomatic deficiency rather than arbitrary numeric targets.

Ultimately, as the Department of Defense moves forward with implementing the protocol, the broader debate over its scope, medical validity, and political motivation continues to unfold. The military now faces the dual challenge of applying this policy effectively while addressing the full spectrum of healthcare needs across its diverse, gender-integrated ranks, ensuring that genuine medical welfare remains the priority.  (Source: BBC)
